Hourly wage plus benefits. Responsible for case management of Veteran individuals/families, connecting Veterans to VA health care benefits in the SSVF Program in Burt, Cuming, Dakota, Dixon, Dodge, Thurston, and Washington counties.

The SSVF Case Manager/Health Care Navigator will work with the SSVF Program Coordinator to ensure all Federal and State regulations of programs are enforced. Work collaboratively with Veteran Affairs Officers in the service area and ensure efficiency of the SSVF programs. Provide outreach on programs offered. Work with referral sources and community partners to identify candidates for the SSVF Program. Work with participants to develop a housing plan with goals for successful achievement of self-sufficiency. Assistant participants with employment services and/or obtain financial assistance from government or private sources. Maintain complete and accurate records, filing systems, and Clarity data entry on all clients. Monitor progress of clients on a daily/weekly/monthly basis by home visits, office visits, telephone, and mail. Connect Veterans to VA health care benefits or community healthcare services where veterans are not eligible for VA care. Attend meetings and trainings as requested. Maintain confidentiality of all participant/client information and follow Program and Agency standards and policies.
EDUCATION & EXPERIENCE:
Bachelor’s Degree in field related to the delivery of counseling or social services preferred. Knowledge of VA resources and benefits a plus. 6 plus months of experience in related field preferred.
WAGE & BENEFITS:
The starting wage is $17.00 per hour, plus benefits. Consideration will be given to education and experience. Benefits include health, life, dental and vision insurance. Paid holidays, vacation, and sick leave, EAP, 401K Retirement.
