The Outreach & Case Management Coach is responsible to develop and oversee an outreach program and advocate and represent the organization and its community purpose. Work with Family Services staff in developing ideas and projects to serve people in the community. Responsible for the development and training of a case management program that measure and report participant outcomes. Monitor and evaluate Case Management client progress and assist clients in reaching their goals. Prepare case management reports and complete reporting requirements meeting CSBG National Performance Indicators. Locate educational information and promote outreach activities. Update appropriate forms and brochures and utilize the Agency data base system. Demonstrate experience in case management and providing support services to diverse populations in an advocacy, education, or case management setting. Work with individuals dealing with stressful or traumatic life events while maintaining appropriate professional boundaries. Maintain confidentiality of all participant/client information and follow Program and Agency standards and policies.
EDUCATION & EXPERIENCE:
Master of Social Work preferred. Six plus months of experience in a field related to the delivery of Counseling or Social Services preferred.
